Meaning of the śloka
A physician entering practice should have studied the treatise, learned its meaning, observed procedures, completed practical training and be able to expound the teaching. The physician should have the king's permission, keep nails and hair trimmed, be clean, wear white clothes, carry an umbrella and staff, wear footwear and dress without ostentation. The physician should have a good disposition, speak auspiciously, avoid deception, act as a friend to living beings and have good assistants.

Meaning of the commentary
The commentary explains the qualifications for entering practice. Studying the tantra means reading the treatise; learning its meaning means studying its interpretation beside a teacher. Observed procedures include cutting and the other operations, and sneha and the other therapeutic procedures. Practical training means repeated practice of the actions. Expounding the teaching in its wording and meaning, in the prescribed manner, indicates continued command of the text. Alternative readings say 'expounding the meaning of the teaching' or 'expounding the treatises.' Royal permission should follow examination: the king must protect the people, and neglect allows impostors resembling physicians to move through the country and trouble the public. The discussion then turns from knowledge and skill to outward appearance. Trimmed nails promote cleanliness and ease of work. Trimmed hair is associated with neat simplicity; others explain that it avoids trouble in the kitchen. Cleanliness here means external cleanliness, such as bathing, because outward appearance is being discussed; inner purity is covered by a good disposition. White clothing is for auspiciousness, or for cleanliness and simplicity. An umbrella offers protection from sun, dust, rain, snakes and similar hazards; a supporting staff prevents stumbling, and footwear protects against thorns and the like. Unostentatious dress means the dress of respectable people and excludes unsuitable bodily presentation. Others explain it as avoiding showiness in varied clothes, body colours, garlands, ointments and elaborate ornaments. A good mind possesses qualities such as freedom from cruelty; despondency is considered undesirable when setting out. Auspicious speech is intended to win the goodwill of all. Freedom from deception restrains body, speech and mind: a kuhaka is a deceiver lacking scriptural learning who nevertheless inspires great trust. Others understand akuhaka as someone who does not seek to astonish through pretentious appearance or behaviour. Good assistants matter because a physician visiting the inner household may incur blame through an assistant's fault; assistants also provide personal protection and patient care. Finally, an objection asks how the physician can be a friend to living beings when fresh blood from living creatures is prescribed. The commentator accepts the point but argues that a practice prescribed by the teaching is not blameworthy, comparing it with killing an animal in a prescribed sacrificial rite.
